Job summary
Sett Valley Medical Centre is looking for an enthusiastic and forward-thinking GP to join our friendly, supportive team on a fixed-term basis to cover maternity leave. The role will start in December 2026 for 9 months, with the possibility of extending to 12 months.
The post is for 6 sessions per week, working Monday, Wednesday and Thursday, including occasional Wednesday evening surgeries until 8pm. The role involves a mixture of face-to-face and telephone consultations within a well-established multidisciplinary team and the wider High Peak PCN.
We benefit from excellent prescribing support through our PCN and in-house pharmacists.

Main duties of the job
As part of our multidisciplinary team, you will provide high-quality, patient-centred care through a varied and rewarding role that includes:
- Face-to-face and telephone consultations
- Management of acute and ongoing patient care, including investigations, results, referrals, and treatment planning
- Developing personalised care plans in partnership with patients
- Reviewing and authorising repeat prescriptions
- Managing clinical correspondence, queries, and other associated administrative work
- Occasional home visits, supported by our well-established home visiting service and community matrons, meaning home visit requirements are typically minimal
You'll be supported by an experienced clinical and administrative team, as well as our in-house and PCN pharmacists, allowing you to focus on delivering excellent patient care.

Job summary
NWL has the largest homeless population in London. This population faces fundamental inequalities in outcomes, experience, and access - with multiple, long term, under-treated health needs and do not always get the care they need from the health and care system.
People experiencing homelessness have six times more A&E presentations, are admitted to hospital four times more often, and stay three times as long as the general population.
In hospital, they are likely to be poorly identified, experience delays once medically fit for discharge, and are at high risk of discharge to the street, readmission and poor overall health outcomes.
We have received confirmation from the NWL ICB that a business case has been approved to fund a new specialist hospital Homeless Health team up until 31st March 2029.
The Homeless Health team works in close partnership with the wards to identify people experiencing homelessness whilst they are in hospital, build therapeutic relationships with them, assist the ward multidisciplinary team to maximise the benefit of these admissions, and then support these vulnerable patients during and after discharge - ensuring that they are fully linked in to all the health care and support services they need when the team finally discharges them.
The GP role works to improve the care for homeless patients in hospital, to drive up the quality of the care they receive during a hospital stay, and to improve their quality-of-life following discharge.
Main duties of the job
Working with the rest of the Homeless Health team, the GP role works to improve the care for homeless patients in hospital, to drive up the quality of the care they receive during a hospital stay, and to improve their quality-of-life following discharge. The key responsibilities of the role are to:
To clinically lead, support and promote the specialist homelessness team in the hospital.
To work collaboratively to improve health and social outcomes for people who are homeless.
To promote and lead weekly multi -disciplinary team meetings for hospital, primary care and voluntary sector staff working with the most challenging patients.
To offer clinical judgment/expertise on how best to address complex issues among this patient group, particularly with reference to the tri -morbidity of substance misuse, mental illness and physical health problems.
The successful post holder will have substantial personal experience of working with people who are homeless and will have a demonstrable special interest in:
Working with people experiencing drug or alcohol dependence or/and domestic violence including substitute prescribing.
Experience of working in or with the wider statutory and charitable homelessness sector.
- Experience of mental illness particularly working with patients with complex psychological trauma/diagnosed with personality disorder
- Sexual health.

This is an opportunity to work for a PCN that has established teams who work well collaboratively. This is a fixed term contract to service the same day acute presentations that increase during the Winter.
The successful candidate would form an integral part of the team providing acute respiratory illness (ARI) clinics. We hope to introduce the successful candidate in to the team prior to the peak winter demand so that they have chance to become embedded in to our team.
Gateshead Inner West already has an established MDT of pharmacists, pharmacy technicians, first contact physios, SPLW and frailty nurse. This position would benefit from access to these services to aid clinical workload. All practices across the PCN endorse a supportive clinical environment and have experienced GP teams available for support.
Main duties of the job
This role has been created to service the increased Winter demand that GP practices face. We want to improve the patient journey for our patients and endeavour to reduce admissions to A&E.
The successful candidate would form an integral part of the ARI clinics. We hope to appoint before the peak winter season, to provide more same day appointments. We would then timetable ARI clinics. Duties would include: -
- Providing same day appointments, to patients triaged as to needing same day review. We would anticipate acute illnesses and would be actively triaging so that presentations requiring ongoing GP follow-up were not booked in to these slots
- As the winter progresses and ARI clinics are commissioned we would anticipate this being the primary clinic type provided by the successful candidate
- The candidate would be expected to be comfortable in dealing with same day acute GP presentations
- We would expect appropriate clinical documentation and coding
- The successful candidate would be expected to order their own investigations and manage onward referrals

An exciting opportunity has arisen for Band 6 practitioners to work within our established Urgent Community Response Team in West Essex.
The teams assess and manage acutely unwell patients at home using a variety of pathways including respiratory, heart failure and frailty, to prevent unnecessary admissions to hospital.

Main duties of the job

St Andrews House Medical Centre are looking for a GP to join us on a 4 sessions per week, 12 months fixed term contract.
The days of working would be Tuesday and Friday.
St Andrews House Medical Centre, Stalybridge (Tameside and Glossop ICB) is looking for a motivated GP who is enthusiastic, forward thinking, and adaptable to working within our well established, friendly, and innovative training practice.
We are seeking an individual who can provide 4 sessions per week of GP work, each session lasting for 4 hours and 10 minutes.
15 minute appointments/patient
Admin/Home visits/Oncall
Main duties of the job
Variety of duties including face-to-face consultations, telephone consultations and queries, consultations using video and photos. Home visits and nursing home rounds.
In consultation with patients and in line with current Practice disease management protocols, developing care plans for health.
Providing counselling and health education as needed as part of the consultation
Referring and doing their own referrals to other care providers as appropriate
Recording clear and contemporaneous consultation notes to agreed standards
Collecting data for audit purposes
Compiling and issuing computer-generated acute and repeat prescriptions (avoiding paper and hand-written prescriptions whenever possible)
Reviewing, filing and acting on documents as stored in the Docman software.
Prescribing and referring in accordance with the Practice policy as determined GM authorities whenever this is clinically appropriate
In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
Provide advice to queries raised by Practice Nurses, Advanced Nurse Practitioner, Health Care Assistant and GP Registrars, while doing a clinical session, if required.

The Adolescent South Team is a motivated and well supported team working across Cambridge City, South Cambridgeshire and East Cambridgeshire. It is at the forefront of our new pathways for children facing risks outside the home, and closely connected with colleagues in the Missing and Exploitation Team and Youth Justice Service. Young people are referred to the team for one or more of these reasons: they regularly go missing; they are at risk of criminal or sexual exploitation; they are involved in serious violence; they have significant challenges with their mental health or substance misuse; they are aged 16/17 and homeless.

What will you be doing?
You will be responsible for a small caseload of approximately 6-8 children aged 12 and above where the primary safeguarding concerns relate to risk outside the home. Most children open to the team are supported through Child In Need Plans, but you will also undertake some Child and Family Assessments and work with some Children in Care and some subject to Child Protection Plans (including through our ROTH CP framework).

The Cambridgeshire Adolescent Service is focused on safeguarding vulnerable young people who are facing risks outside the home. We achieve that by building:

- Connections with community resources that can move children towards positive futures.
We have an exciting opportunity for an experienced social worker to join the South Adolescent Team as aSenior Practitioner on a full time, 12 month, fixed term contract.
The Adolescent South Team is a motivated and well supported team working across Cambridge City, South Cambridgeshire and East Cambridgeshire. It is at the forefront of our new pathways for children facing risks outside the home, and closely connected with colleagues in the Missing and Exploitation Team and Youth Justice Service. Young people are referred to the team for one or more of these reasons: they regularly go missing; they are at risk of criminal or sexual exploitation; they are involved in serious violence; they have significant challenges with their mental health or substance misuse; they are aged 16/17 and homeless.
