Collectively we deliver health care services to approximately 65,000 registered patients.
WHA South is a forward thinking and supportive PCN that encourages personal development and growth. We are committed to improving patient care with a particular focus on health inequalities across the Network footprint.
The PCN team currently consists of Nurses, Pharmacists, Technicians, Care Co-ordinators, Physiotherapists, HWCs and MHPs funded under the Additional Roles Scheme. We are looking to recruit an experienced Nurse to join our community team for a fixed term initially with the potential to become permanent.
The WHA CaCHT operates across the PCN footprint, under the guidance and support of the lead ANP and GP, delivering enhanced care to the frail and elderly patients as outlined in our Neighbourhood Health Plan. We have a specific focus on LTC's and more specifically Dementia, and work with local community services to support both patients and families where appropriate offering an holistic approach to care.
