Job summary
An exciting opportunity has arisen for Band 6 practitioners to work within our established Urgent Community Response Team in West Essex.
The teams assess and manage acutely unwell patients at home using a variety of pathways including respiratory, heart failure and frailty, to prevent unnecessary admissions to hospital.
This includes the utilisation of remote technology and new ways of working to provide a safe alternative to hospital admission and support people to be discharged from hospital earlier than may have been possible previously. The focus of this role is delivering high quality care in the community, holistic assessments, planning, implementing, and reviewing care provision across the Hospital at Home and UCRT pathways of care. This is an exciting opportunity to shape services that will improve the experience of healthcare for the population of West Essex and deliver an NHS national priority.
We are looking for enthusiastic and motivated individuals to join the team with a range of skills and clinical expertise. You need to be flexible, hardworking, have excellent communication skills and appreciate the need for professional development within your practice to ensure we establish a responsive and effective team.
Main duties of the job
- Responsible for prioritising and planning own workload on a day-to-day basis with some direction.
- To use advanced knowledge and skills in order to manage patients care as an autonomous practitioner; without direct supervision, drawing on advanced theoretical and practical experience.
- Negotiate, agree expectations, and set appropriate realistic and individual goals in partnership with the patient, carer and colleagues
- Ensure that assessments, programmes of care, and evaluations related to patients being accepted and onboarded onto the service pathways comply with the Trust's agreed clinical standards.
- Ensure provision of holistic and rapid assessment of patients referred in a timely manner to support early discharge from the acute hospital provision.
- Provide home visits and deliver programmes of care according to clinical need within agreed protocols after escalation and discussion at MDT meetings.
- Facilitate safe and timely discharge from the service including generation of a discharge summary to GPs.

MAIN RESPONSIBILITIES
- The post holder will be a registered nurse or Paramedic who has extensive clinical knowledge. You will have experience at working at a band 6 level and have acute and/or community experience. - As the Senior OT, you will have responsibility for the OT provision on your ward, which includes managing clinical interventions using the VdT Model of Creative Ability, as well as the supervision of junior OT staff.
- To work as an autonomous professional according to the Royal College of Occupational Therapy 'Professional standards for occupational therapy practice, conduct and ethics (2021)' and the 'Health Professions Council Regulations'.
- To contribute to the overall service development of the team and actively participate in Continuing Professional Development. Provide leadership through supervision, mentoring, education and training.
- To work as part of the integrated team to plan, deliver and evaluate all aspects of the patient treatment process, including discharge and referral to appropriate community based health and social care agencies.
Main duties of the job
1. To assess clients referred to the service.
2. To utilise specialist assessments when presented with complex cases.
3. To make decisions on the level of treatment intervention required.

Job summary
We have a half time vacancy for an Assistant Psychologist in our South East Essex ARMS (At Risk Mental State) for psychosis Services in Essex. We are a standalone multidisciplinary team with psychological staff, peer support workers, a recovery coordinator and an employment and education specialist. We have been operational for just over four years now and we continue to develop and update our processes and ways of working. We take a psychological approach to all aspects of our service.
We cover Southend, Rayleigh, Castlepoint and Canvey Island. We offer flexible working and remote work is possible for some of the role. It is an interesting and diverse role which will provide the successful applicant with broad experiences and good learning opportunities in both clinical, audit and service development tasks. We are looking for someone who is friendly, hardworking, adaptable with excellent organisational skills. We are one of three ARMS teams in the Mid and South Essex area and we do a lot of joint development. We are also closely connected to our local Early Intervention in Psychosis team.
Main duties of the job
The role of the Assistant Psychologist is to assist with all parts of the Psychological Service provision, including individual and group assessments and interventions, supporting projects such as service user involvement, therapy or carers support, administrative and organisational tasks, audit and service evaluation. The role also includes working with and supporting all members of the multidisciplinary team where appropriate.
We are looking for a graduate in Psychology who has some relevant experience working in health and social care settings. The post holder will need to be organised in order to assist the psychology team in managing referrals and collecting and evaluating outcomes of interventions as well as having creative skills for producing documents and resources that are part of our ongoing development as a service. There will be opportunities for involvement in work that cuts across the other MSE ARMSp teams.
